Document Requirements

1. Computerized Hospital Bill / (Admitted)
2. Medical Certificate or Certificate of Confinement with Hospital Logo (Remarks: “ Emergency Cases”)
3. Barangay Certification of Voter’s Certificate
   • for Senior Citizens – certificates of their children should be submitted
   • for Minor Age – certificates of professional writer service parent should be submitted
4. Laboratory Result signed by Medical Technologist ( For Dengue Patient)
5. Affidavit of Support for unrelated persons

FOR PRIVATE HOSPITALS > REQUIREMENTS FOR LABORATORY REQUEST (MRI, ENDOSCOPY, RADIATION, CHEMOTHERAPY, ETC.):

1. Personal Letter address to Mayor Inday Sara Duterte
2. Barangay Certification or Voter’s Certificate
   • for Senior Citizens – certificates of their children buy essay should be submitted
   • for Minor Age – certificates of parent should be submitted
3. Computerized Quotation or Statement of Account from the Hospital
4. Certification from Mrs. Nanet Macapundag- Lingap SPMC (if laboratory request is not available at SPMC)

FUNERAL FINANCIAL ASSISTANCE

Document Requirements

1. Original Contract ( Funeral Bill)
2. LCR- Certified true copy of Death Certificate
3. Barangay Certification or Voter’s Certificate
   • for Senior Citizens – certificates of their children should be submitted
